The role involves cross working over three pathways incorporated within the Rapid Access Service: the Rapid Response Team, the Rapid Early Discharge Service (REDS) and the Post-Acute Care Enablement (PACE) team, all based at St. Pancras Hospital near vibrant Camden Town, but covering the whole of the London Borough of Camden, one of the most diverse populations in London. The service is purely community based and has the privilege of being invited into the homes of Camden residents from as far north as the borders of Golders Green and as far south as Holborn and everywhere in between.

The Rapid Response Team care for patients who are acutely unwell and at risk of admission to hospital and require a timely, same day, intervention to provide acute nursing and therapy intervention to prevent that admission. The PACE Team facilitate early discharge from the Royal Free Hospital for patients who still require on-going medical, nursing and acute therapy intervention. REDS facilitate early discharge from Camden’s local hospitals for patients who are medically fit for discharge and facilitate reablement-led discharges for patients who are below their functional baseline. 

All three teams work together to act quickly, efficient, comprehensively and always with the patient at the centre of any decision making with an agenda to reduce hospital bed days and holistically care for the patient in the best place possible; their home. Experience of working autonomously is essential as our team is often faced with some challenging situations which present themselves in the home environment, but as an occupational therapist, this truly is the best place to assess someone’s functional performance. 

You will provide rapid specialised assessment and treatment of patients who have complex physical, psychological and sensory impairments and initiate services to safely avoid unnecessary hospital (re)admission. You will have a leadership role in the day-to-day operational management of junior staff, delegating activities appropriately and monitoring the quality of care given. You will be expected to identify and promote areas for service development and support the therapy lead in the day to day running of the team. As the service is seven days, occasional evening and weekend working is necessary.

Regular CPD sessions and in-services are held and along with training opportunities identified from yearly appraisal, the Rapid Access Service therapists also share a service line with Palliative Care so the benefits and collective knowledge bases of working in a bigger group can be experienced and shared.

You are a senior, experienced occupational therapist who understands your role and contribution to the multidisciplinary team environment and has the proven ability to communicate in an exceptional manner. You possess a broad knowledge base covering a variety of inpatient and community specialities that can be transferred to this type of care delivery.
