ROLE OVERVIEW
The Clinical Case Manager will be motivated and innovative with strong leadership, management and communication skills to ensure that the care delivered is safe effective and reliable.
The Clinical Case Manager will also assist as part of the wider team in the development and delivery of our complex care services.
Working with people of differing ages and with a variety of clinical needs, they will assess, plan, implement and evaluate evidence-based, person centred care. Specifically they delegate safely and appropriately and supervise team members, carry out and supervise clinical nursing procedures and collaborate with professionals across agencies and sectors.
Key Responsibilities
* To take full accountability for management of a clinical case load in the community setting
* To train staff on clinical and delegated nursing tasks, support and motivate team members.
* To organise and prioritise workload of the team by working alongside the care coordinator for each area.
* To provide back up and " Buddy" support for your clinical team colleagues
* To assess, plan, implement and regularly evaluate the clients' holistic care in partnership with the multidisciplinary team, ensuring that the documentation is maintained and updated.
* To maintain accurate and contemporaneous records in line with local policies and NMC guidelines
* To develop training plans for each person we support and undertake competency training for individual staff members as required.
* To identify the training needs of care staff and direct, coordinate and supervise the development of their competency and professional skills through teaching, mentoring and support.
* To promote the development of team members by identifying their training and development needs, facilitate self-appraisal and providing performance review.
* To undertake clinical audits and be involved in developing and leading service improvements.
* To provide Clinical Supervision on a regular basis to designated staff members.
* To conform to NMC requirements by keeping up to date with current trends and developments in nursing and to demonstrate evidence of such.
* To ensure nursing practice is in accordance with the NMC Codes of Conducts
* Maintaining a flexible working pattern to meet the needs of the business. This means that you will be asked to work a more flexible shift pattern so that we can maintain delivery of our services in the evenings or at weekends
* To be involved in recruitment and selection of staff.
* To participate in clinical governance and risk management procedures in line with policies and guidelines.
* To be aware of the Voyage Care Complaints Policy and be able to take appropriate action and implement and undertake investigations as per process.
* To participate in a shared on call rota alongside coordination staff
