Corus Consultancy are looking for enthusiastic, motivated and qualified Support Worker for our client in North Yorkshire. This is a temporary contract position for 3 months with the possibility of extending the contract.

Working with our client in North Yorkshire you will be experience working in a changing enviroment, the post holder will asssist the Directorate in meeting its aim of closer working relationships with Health and other partners to deliver improved outcomes for vulnerable adults.

Main Duties:

* Undertaking assessments of need with vulnerable people as allocated by the Team Manager and where requested, to undertake assessments of need with carers.
* Providing professional support and advice to people and carers on how their needs could be partly or wholly met by access to universal and other non-care services.
* Intervening in emergency situations to protect vulnerable people and to initiate the appropriate statutory and other actions required.
* Maintaining clear, concise and timely records of cases, care pathways and actions in line with the Directorate's policies on file maintenance.

What we require of you:

* Social Work Degree or equivalent qualification
* HCPC Registered
* Enhanced DBS Check
* Experience of inter-agency collaboration practice
* Demonstrable experience of undertaking assessment of health or social care needs in a community setting (Newly Qualified workers only)
* Demonstrable experience of contributing to the safeguarding of vulnerable adults (Newly Qualified workers only)
* Demonstrable experience of initiating the appropriate statutory and other actions required to undertake adult protection investigations
* Relevant community experience for nursing qualified applicants
