Looking for a BAND 8 Lead Pharmacist

The Lead Pharmacist is a new role within this particular service. The postholder will have the lead role in driving forward the medicines optimisation agenda by developing and delivering a clinical pharmacy service to improve patient outcomes including to have enabled a range of multidisciplinary practitioners within the team to optimise the use of medicines within their scope of practice.

MAIN RESPONSIBILITIES:

* To support the introduction and implementation of an assessment process to support service staff to identify patients under their direct care at the highest risk of medicines related morbidity and / or hospital admission or re-admission that need pharmacy input.
* To undertake a holistic medication review of identified patients in either the proactive clinic or home environment, to agree a care plan with the patient and carer and to make recommendations to the various health and care professionals to optimise the use of medicines.
* To carry out simple practical interventions to support the patient to take their medicines as prescribed.

Pharmacy Technician Requirements:

* Start Date - ASAP
* CRB/DBS dated within a year
* Relevant Qualification / GPhC
