Were now looking for a Care Coordinator to join us in our re-ablement service, based out of Camberley, to assist in the day-to-day operation of the Home Care Service business, involving direct line management for a team of Home Care Workers supporting people who have come home from hospital, enabling them to get back to full health or to become as independent as possible.
The hours of work will be 8am to 5pm - Monday to Friday and is initially a 6-month contract.
* **In the role, you will:** *
* Participate in the multi-disciplinary assessment of Service User needs and the subsequent planning of Service User Care.
* Support hospital discharges
* Managing the weekly load of the Care Workers to ensure that services are delivered in line with contracts
* Defining any special care needs and liaising with external organisations (CPNs, chiropodists, GPs etc.) to develop a total package of care
* Report into the Branch Manager on a weekly basis on activity levels and outcomes for service user
* Engage with and positively undertake on call duties as required by the branch to ensure that a 24/7 service is provided.
* Participate in staff development, training and appraisals and provide written reports of all complaints, accidents and incidents
* **To be successful you will:** *
* Have a full Driving Licence and access to a car
* Hold or be willing to undertake an NVQ/SVQ level 3 in Care
* Have previous experience within a Care environment
* Have previous experience of coordinating and managing a team
* Have excellent spoken and written English, good IT skills, competence in Microsoft Office and possess strong leadership skills.
* excellent communication and interpersonal skills
* Health & Safety / First Aid / Food Hygiene / Moving & Handling certificates would be an advantage, as would experience with Cold Harbour systems, care plan writing and accident reporting.
