Position: Senior Care Support Worker

Responsible To: Deputy Operational Manager

**Responsible For: ** Responsible for mentoring care support workers, dealing with breakdowns in the community, starting new care packages, completing observations and supervision in the community to ensure a safe, effective and quality caring service is delivered.

JOB PURPOSE
----------------------------------------------------

* Deliver a safe, effective, quality and caring service, ensuring all tasks are carried out in line with the needs, wishes and preferences of the individual, following the care plan.
* Provide direct personal/domestic care in accordance with the service users assessed needs, for example:-
* Assisting/supporting with personal hygiene and appearance, i.e. toileting, bathing
* Preparing/cooking meals and assisting with feeding where necessary
* Undertaking routine domestic and household tasks
* Shopping, collecting pensions and prescriptions
* Prompting/administrating medication under the direction of medical professionals and/or manufacturers guidance
* Deliver specialised care services, i.e., Stoma, Peg and Catheter care
* Act as a role model to care support workers, promoting best practice in all duties.
* Encourage and support care support workers, providing advice, support and feedback to the care support workers and office.
* Respond and attend to any breakdowns of care delivery/emergencies in community.
* Respond to and attend new care packages, including a handover of the service to care support workers.
* Complete medication audits, observations and spot checks in the community.
* Mentor new team members through the initial 12 week induction period.
* Develop positive relationships with service users, family members and other team members.
* Contribute to/attend service user review meetings.

* DELIVERING SPECIALISED CARE SERVICES; STOMA, PEG AND CATHETER CARE
